Mission & Vision
Health & Safety: Maintaining the health and safety of the persons we support is our number one priority. We have several quality assurance measures including adhering to the American Physician Association guidelines for screenings and evaluations for persons with intellectual disabilities, and closely monitor needs with a comprehensive electronic healthcare records management system. We also consult with an independent registered nurse who audits medication storage, administration procedures, and training.
Community Inclusion: We define a high quality program by our ability to offer as many opportunities as possible to participate in the community. The persons we support are immersed in the community as much as they wish in an effort to create community connections leading to natural supports and feelings of belonging and worth.
Communication: Community Concepts, Inc. believes in administering a transparent program through open, honest and frequent communication with family and stakeholders. We encourage the active family involvement and provide opportunities for involvement through the development of family support groups and stakeholder satisfaction surveys. No one can care for your family member like you can and we respect that and value you as a team member.
Dignity: We create an environment where supported persons can voice their opinions and exercise control over their lives, and advocate for themselves. For example, supported persons take part in the selection of their homes and common area furniture when possible. Personal choice of supported persons can be seen in their bedroom decor, menu selection, and their house leisure and recreational activities. Each site also facilitates house or program meetings where persons in the program have a place to voice their opinions.
